I hereby declare I am the parent or legal guardian of the child and have the right and authority to enroll this child in language classes at the International School of San Antonio (ISSA).

I agree that the participation in ISSA classes is completely voluntary and based upon an informed decision.

I understand that my child will be given the opportunity to participate in physical and group activities that will be supervised by the trained staff members of ISSA and the staff will make every effort to ensure my child’s safety. However, I acknowledge that physical and group activities carry an inherent risk of injury and I hereby release ISSA, Oak Hills Presbyterian Church, and their agents, employees and directors from any and all liability should my child become injured while participating in ISSA’s activities.

This is a full and final release and waiver of liability and all claims that I, or my child, may have now or in the future, against ISSA or Oak Hills Presbyterian Church, and any persons associated with or otherwise participating in activities at ISSA or Oak Hills Presbyterian Church, in any capacity from any and all liability for any loss, damage, injury or expense that I or my child may suffer as a result of the use of, or presence at, Oak Hills Presbyterian Church and ISSA, and any claims now or in the future that I or my child may have due to any cause whatsoever, including negligence, gross negligence, breach of contract or breach of any statutory or other duty of care, including any duty of care owed under any liability act or theory.

I understand that should my child become injured or ill, the ISSA staff will take immediate action to help my child, including, but not limited to, administering basic first aid, administering basic CPR, calling and reporting 911 life-or-limb-threatening emergency, and that by signing this form, I have given my express consent to do so. I also understand that as soon as possible following my child’s accident or illness, an ISSA staff member will contact me. I agree to leave an emergency phone number so that the ISSA staff may contact me if needed.

I understand that any special medical conditions my child has must be provided in writing, with appropriate instructions should a medical situation arise, and that emergency medications may only be administered with a physician’s note. I understand that ISSA will be providing food and beverage to my child throughout the day. I understand that any special food allergies or requirements my child has must be provided in writing with appropriate instructions, and shall provide substitute/alternative food or snacks.

By signing this Childcare Consent and Waiver, I acknowledge that I have read and understand it, and that I am in agreement with its provisions.

Should the need arise the following emergency treatment procedure will be followed:

A trained and certified staff member will take whatever immediate steps necessary to stabilize the child’s condition.
If further care is needed, appropriate medical personnel will be contacted (i.e. 911)
Staff will notify Parent/Guardian of situation as soon as possible.


It is imperative that all enrollment forms contain current contact information, as well as emergency medical information and authorization for medical treatment, if applicable.

The classes drop-off window is 8:50-9:00 AM. If you are late, please send us an email to info@issatx.org or call us at 210-920-4315.

The pick-up time is 11 AM.

If parents/caregivers are more than 15 minutes late, they will be charged a late fee of $15 for each additional 15 minutes (or part thereof).
